Fetal echocardiography is a specialised ultrasound of the developing baby's heart. While a routine anomaly scan includes a basic 4-chamber view, fetal echo goes much deeper — evaluating all cardiac structures, valves, great vessels and flow patterns using Colour Doppler and pulsed wave Doppler.
🔍 When is Fetal Echo Recommended?
- Family history of congenital heart disease (CHD) in a parent or previous child
- Abnormal findings on routine anomaly scan (4-chamber view, outflow tracts)
- Maternal diabetes (pre-gestational or gestational — increases CHD risk significantly)
- Maternal autoimmune disease — lupus, anti-Ro/La antibodies (risk of fetal heart block)
- Maternal infections — rubella or CMV in early pregnancy
- Fetal chromosomal abnormalities — Down syndrome, Turner syndrome
- Increased nuchal translucency (NT) on NT scan
- Monochorionic (identical) twin pregnancy
- Fetal arrhythmia detected on routine scan
- Maternal teratogenic medications — anti-epileptics, lithium, NSAIDs

4-Chamber View
Size and position of all four heart chambers — atria and ventricles.
Outflow Tracts
Ventricular outflow tracts, aorta and pulmonary artery — major vessel anomalies.
Septal Defects (VSD/ASD)
Ventricular and atrial septal defects — the most common congenital heart defects.
Valve Assessment
Tricuspid atresia, pulmonary stenosis, aortic stenosis — valve morphology and function.

Preparation Guide
- Abdominal ultrasound: Fast 6–8 hours. Water is allowed.
- Pelvic ultrasound (women/men): Full bladder — drink 4–6 glasses of water 1 hour before. Do not urinate.
- Obstetric scans (1st trimester): Full bladder preferred. TVS available if needed.
- Obstetric scans (2nd/3rd trimester): No special preparation required.
- Fetal Echo: No preparation needed. Best performed at 22–24 weeks.
- Thyroid, Breast, Scrotal: No preparation. Wear loose, accessible clothing.
- Doppler studies (vascular): No preparation. Avoid tight clothing over the area.
- Renal Doppler: Drink plenty of water beforehand for good kidney visualisation.
